Pharmaceutical care in palliative care and oncology: experience report in a philantropic hospital nursery

Abstract

Introduction: Pharmaceutical practice in Oncology and Palliative Care is still referred to the hospital environment, but its contribution is essential in the terminal stage. The scope of this professional transcends the rationalization of therapy, meeting the patient's needs focusing on quality of life. Objective: To present an experience report of the performance of a Resident Pharmacist in the pharmacotherapeutic follow-up of cancer patients and in palliative care. Apresentation of Professional Experience: Descriptive study, of the type report of professional experience, through the experience in a palliative care and oncology ward of a philanthropic hospital in the city of Salvador - Bahia. Discursion: During the residency period, the professional experiences the care routine under the supervision of the Clinical Pharmacy Service. The actions taken were: pharmaceutical consultation, pharmacotherapeutic follow-up, pharmaceutical interventions, assessment of adherence and toxicities or adverse events, cancer pain management and participation in multidisciplinary visits. This set of actions aimed to meet pharmacotherapeutic needs and promote strategies to improve the quality of care provided to patients. From a professional point of view, the actions promoted the construction of a critical look in comprehensive care, enabling the reconfiguration of practices and attitudes based on the care experience. Final Considerations: The experience period promoted an understanding of the role of the pharmacist in the therapeutic itinerary of these patients. The residency program offers specialized assistance to health service users and these, in turn, contribute to the improvement of residents.
